How many grafts do I need?

That is a big confusion for everyone looking for a hair transplant.

Infact, its almost impossible to say accurately how many grafts for hair transplant one needs. So lets look at the factors that decide your graft requirement.

1. The no 1 factor that decides your graft requirement is your grade of androgenic alopecia. On an average

  • Grade 2 : 1000 to 1500
  • Grade 3: 2000 to 2500 per area. Le frontal area will need these many grafts. Crown area will need these many grafts more.
  • Grade 4: 3000 to 4000
  • Grade 5: 4000 to 5000
  • Grade 6: 6000 to 7000
  • Grade 7: 8000 to 10000.

All these are the average number of grafts needed for hair transplantation according to a density of 35 to 45 grafts per cm sq.

2. The second important factor while deciding hair transplant grafts is the density we are looking for. Natural density that is 80 to 100 grafts per cm sq. however we now know that only when half the hair is lost in an area, only then we are able to see hair thinning. So  logically, we don’t need to replace 80 to 100 grafts per sq cm. we only need to replace 40 to 50 per sq cm if we want to achieve a good look and excellent coverage. However, if someone demands for very high density, and his donor area and stage allow for that, then we can go upto 60 to 70 grafts per cm sq as well.

3. The third important factor for hair grafts is the presence of hair in that area. Many a times, the area that we are planning to transplant already has some native hair that are not completely lost. They are just thinner, but can grow and thicken with medications and regenerative therapies. In that case, the graft requirement reduces. or if we decide not to reduce the graft no, the final density of transplant will be higher.

4. Another factor is the size of the head in general. 2 people with same grade of alopecia may have different area to cover because of differences in their head size. For eg Broader foreheads require more grafts to create hairline.

5. Fifth important factor for number of hair grafts is the availability of donor area, now and for future. If we analyse and understand that a person will need more hair transplants in the future, then we have to choose graft no. not just according to his current need but also for future. We may choose a lower graft no for him or her today so as to save hair for the future.
